    Matthew S. Durstenfeld, MD, MAS, is a physician-scientist focused on how infections contribute to cardiovascular disease in humans using observational studies and mechanism-oriented clinical trials. Because the cardiovascular system is a dynamic responder to stressors including from infections, he developed expertise in probing human biology through exercise, specifically cardiopulmonary exercise testing (CPET). Cardiorespiratory fitness and heart rate responses during exercise are strongly predictive of future cardiovascular disease and mortality, providing further rationale for this focus. As a post-doctoral T32 fellow and early faculty on a K12 career development award, he studied the cardiopulmonary phenotype of Long COVID, resulting in international presentations and high-impact publications. Supported by a Center for AIDS Research Mentored Scientist Award, Dr. Durstenfeld used CPET to compare cardiorespiratory fitness in Long COVID and HIV, finding that chronotropic incompetence, or inability to increase heart rate during exercise, was common in both conditions. After externally replicating this finding in HIV, he wrote a successful K23 career development award focused on cardiorespiratory fitness and chronotropic incompetence in HIV. Under mentorship of Priscilla Hsue, Dr. Durstenfeld is involved in several mechanism-oriented clinical trials in HIV focused on lipid-lowering and anti-inflammatory strategies to better understand mechanisms that contribute to atherosclerosis in HIV. His work in Long COVID has shifted toward running the CPET core lab for a multicenter mechanism-oriented clinical trial of baricitinib, a JAK-STAT inhibitor, to test the hypothesis that immune dysregulation contributes to Long COVID symptoms. He has also expanded his portfolio to post-tuberculosis heart and lung outcomes using echocardiography and CPET to identify mechanisms contributing to post-tuberculosis phenotypes in Uganda. By understanding how infections drive cardiovascular disease using exercise as a probe, Dr. Durstenfeld hopes to identify targetable mechanisms to mitigate excess cardiovascular risk from infections and prevent cardiovascular disease more broadly.
